Estimate of Costs
- Atlantic Surgery Center provides each patient with a personalized estimate of anticipated facility charges specific to their scheduled procedure prior to surgery.
- Estimates or updated estimates are also available upon request by the patient, prospective patient, or legal guardian.
- Please be advised that an estimate is based on the information available at the time it is prepared, and actual costs may exceed the estimated amount.
- Patients are encouraged to contact their health insurance carrier or HMO directly regarding coverage, deductibles, copayments, coinsurance, and other anticipated cost-sharing responsibilities.
- Atlantic Surgery Center will provide an itemized statement of charges upon request by the patient or the patient’s legal representative.

Collections & Payment Process
- Prior to the day of surgery, our staff is available to provide patients with an estimate of anticipated facility charges, available insurance benefit information, and the estimated amount that will be the patient’s responsibility.
- Atlantic Surgery Center will bill all valid health insurance carriers.
- We will bill the patient’s primary and secondary insurance carriers, or applicable governmental agency, directly for facility charges.
- Any estimated patient responsibility, including applicable deductibles, copayments, and coinsurance, will be collected at the time of surgery.
- After your insurance carrier has processed the claim and issued an Explanation of Benefits (EOB), an itemized statement will be mailed for any remaining balance determined to be the patient’s responsibility.
- Atlantic Surgery Center accepts cash, cashier’s checks, credit cards, and personal checks with a valid government-issued photo ID.
- As an alternative payment option, CareCredit offers financing and payment plan options for eligible patients. Patients may apply online or contact CareCredit directly for additional information and assistance with the application process

The service bundle information is a non-personalized estimate of costs that may be incurred by the patient for anticipated services and that actual costs will be based on services actually provided to the patient.

Other Charges
Services may be provided in this health care facility by the facility as well as by other health care providers that may separately bill you. You will be separately billed for the following:
- Physician fees – from physician performing surgery/procedure
- Anesthesia Fees – from Anesthesia Group providing anesthesia services
- Pathology fees – if any biopsies or specimens were taken
- Radiology Fees – if any images were taken during the procedure
- Drug Fees – if any drugs administered are not covered by your insurance plan
Patients and prospective patients may request from this facility and other health care providers a more personalized estimate of charges and other information. Patients and prospective patients should contact each health care practitioner who will provide services in the ASC to determine the health insurers and health maintenance organizations with which the health care practitioner participates as a network provider or preferred provider.
